Bluecrest Health Monitor (BHM) — Consumer Subscription Terms and Conditions

These Terms and Conditions apply to your Bluecrest Health Monitor subscription. Please read them carefully before you subscribe.

  • Who these terms apply to: These terms apply to consumer customers (not business customers).

  • Territory: We offer BHM to customers in England, Wales, and Scotland only. We do not accept subscribers outside these territories and we exclude sanctioned countries.

  • Governing law and venue: These terms are governed by the laws of England and Wales, and the courts of England and Wales shall have jurisdiction.

What your BHM membership includes

Membership benefits and additional tests

  • Your BHM membership includes up to two additional tests per membership year from our qualifying list.

  • A membership year is defined as a continuous 12-month period of active membership.

  • Unused additional tests do not roll over into subsequent membership years and have no cash value.

  • The qualifying list of additional tests may be changed, amended, added to or withdrawn at any time.

  • Any discounts available to members on additional products or services are provided at our discretion and may be amended or withdrawn at any time.

What your BHM membership includes

  • Access to the BHM digital membership portal and mobile app for viewing your results.

  • Digital access to your results, including downloadable PDF results reports.

  • In-person health assessment(s) and results processing.

  • Up to two additional tests per membership year from our qualifying list.

  • Access to member pricing on selected additional products and services during your active membership.

  • Home test kits, where applicable.

  • Digital content ownership: All content is owned by Bluecrest. You receive a personal, non-transferable right to access and use it for your personal use only. You must not copy, share beyond personal use, reproduce, resell or otherwise use it for commercial purposes. Trademarks are owned by Bluecrest.

  • Naming clarity: "Health Monitor" refers to this BHM membership. "Subscribe and Save" is a separate instalment payment option for standard one-off assessments and is not BHM.

Eligibility and availability

  • Age eligibility: You must be aged 18 to 88 to be a member.

  • Territory and sanctions: Membership is available to customers in mainland Great Britain only (England, Wales, and Scotland). Customers in Northern Ireland, the Republic of Ireland, other countries, and sanctioned territories are not eligible for membership.

  • Personal membership; non-transferable: Your BHM membership is personal to you. You must not assign, transfer, novate, gift, share or otherwise make your membership (or any rights, benefits, appointments, discounts, results, reports or access credentials) available to any other person. We may require proof of identity at appointments and may refuse to provide (or may suspend) services where we reasonably believe the person seeking to use the membership is not the member. Any attempted assignment or transfer in breach of this clause is void and may result in suspension or cancellation of your membership (without affecting any refund rights you may have under these Terms or applicable law).

  • Clinical suitability: Your eligibility to subscribe to (and remain subscribed to) BHM is subject to you meeting our clinical safety and suitability criteria (as described on our website and/or as notified to you from time to time). We may ask you to provide information (including health information) to assess clinical suitability before you subscribe and/or at any time during your membership.
    - If we reasonably determine (acting in accordance with our clinical policies and procedures) that you are not clinically suitable for BHM, or that it would not be appropriate or safe for us to provide BHM to you, we may refuse your subscription application or cancel your membership by giving you notice. Where we cancel for clinical suitability reasons, we will refund any amounts you have paid for services not yet provided, subject to any deductions permitted by law for services already provided (for example, where you have already attended an appointment or received results). Your statutory rights are not affected.

Subscription term, tests, and automatic renewal

  • Minimum term: 12 months from your subscription start date.

  • Rolling after the first 12 months: Your membership continues month-to-month unless you cancel by giving 3 months' notice.

  • Free test entitlement: Your selection of free tests from our qualifying list must take place when you take out your membership. Free tests for subsequent years must be selected around your membership anniversary date.

  • Automatic renewal and notices: After the initial 12-month term, your membership continues monthly unless cancelled with 3 months’ notice. We will give at least 3 months’ notice of your upcoming renewal, and any price change before your anniversary.

  • Opt-out: You may cancel as set out in section 10 (Cancellation).

Prices, taxes and payments

  • Price: From £40 per month.

  • Billing cycle: Monthly in advance.

  • Delivery charges: No delivery charges apply for included items.

  • Payment methods: Card processed via Stripe or Direct Debit.

  • Price increases: May occur on your membership anniversary with at least 3 months' advance notice.

  • Failed payments: If a payment is missed, we may suspend access to the digital platform and cancel booked appointments after contacting you about your membership status. We may elect to charge a late fee for missed or failed payments.

  • Payments will be taken on a monthly basis, payment date is dependent upon your original sign-up date.

Physical items (if provided)

  • Items: Home test kits may be supplied.

  • Delivery: Delivery to England, Wales, Scotland only; estimated 3-5 days.

  • Returns and refunds: No returns for health-related items; where applicable the customer pays any return postage; refunds, where due, are processed via your original payment method.

Digital services and access

  • Access: Online membership portal and mobile app access to your results; you may download your PDF results report.

  • Licence and ownership: All digital content is owned by Bluecrest; personal use only; no sharing beyond personal use, copying, resale or commercial use.

  • Technical requirements: Multi-factor authentication is required to log in; you are responsible for suitable devices and connectivity.

  • Access suspension: We may suspend access for missed payments or prohibited behaviour.

Minimum device requirements

  • To access and use the app, your device must meet the following minimum operating system requirements:

iPhone/iPad: iOS 14.0 or later

Android devices: Android 6.0 (Marshmallow) or later (API Level 23)

  • We cannot guarantee the app will function correctly on devices running earlier operating system versions.

Appointments and service delivery

  • Included appointments: In-person health assessment(s) and results processing form part of BHM.

  • Add-ons: You may purchase additional products/services at any time at a discount, paid at the time of purchase.

  • Service changes and discontinuation: We may change the service, remove content, substitute products or discontinue the service on at least 3 months' notice, or less where urgent for safety or quality.

  • Timeframes to complete Baseline Assessment and Interim Assessment: We recommend that you complete your Baseline Assessment within the first 2 months of your membership start date. Your Interim Assessment is intended to take place approximately 6-9 months after your Baseline Assessment allowing time before the end of your initial 12-month membership term.

  • Appointments are subject to our Terms and Conditions of Supply, available here.

Cancellation and your right to change your mind

  • Cooling-off period (your right to cancel within 14 days): If you are a consumer, you have a legal right to cancel your BHM subscription within 14 days of the day after you sign up (the "Cooling-off period") and receive a refund, subject to the terms below.

Important - attending your first appointment during the Cooling-off Period. If you choose to book and attend your first appointment during the Cooling-off Period, you acknowledge and agree that, once that first appointment has taken place, your right to cancel during the Cooling-off Period will end and you will no longer be entitled to cancel for a refund under the Cooling-off Period.

If you cancel during the Cooling-off Period before your first appointment takes place, we will refund you in accordance with these Terms (including any non-refundable Deposit, except where a refund is required by law). Your statutory rights are not affected.

  • Digital content immediate access: We do not obtain a waiver of the 14-day right for digital content; however, your right to cancel within 14 days ends if you have your first appointment within that period.

  • Cancelling after the minimum term: You may cancel after the first 12 months by giving 3 months' notice. You can avoid moving to a rolling term by cancelling 3 months prior to the end of the minimum term. Examples are provided below, for information purposes only.

Example timeline (12-month minimum term + 3 months' notice after that)
- Start date: 15 January 2027
- Minimum term ends: 14 January 2028 (first 12 months)
Scenario A - cancel at the earliest point after the minimum term
- 15 January 2028: You give notice to cancel (after the minimum term has ended).
- Cancellation effective date: 14 April 2028 (three months' notice runs from 15 January to 14 April).
- Payments: You continue to pay the monthly subscription fees due in February, March and April 2028 (in line with monthly billing in advance).
- Access/services: You keep access and membership benefits until 14 April 2028.

Scenario B - cancel later
- 2 March 2028: You give notice to cancel.
- Cancellation effective date: 1 June 2028.
- Payments: You continue to pay the monthly subscription fees due in April, May and June 2028 (in line with monthly billing in advance).


Scenario C - cancel before minimum term ends
- 1 September 2027: You give notice to cancel.
- Cancellation effective date: 14 January 2028 (end of minimum term).
- Payments: You continue to pay the monthly subscription fees due up to the end of the minimum term (October, November, December).
- Access/services: You keep access and membership benefits until 14 January 2028.

  • Mid-cycle cancellation effects: If you cancel mid-cycle, the remaining balance for the minimum term will be due either immediately (and access will be revoked immediately) or monthly payments continue up to month 12 and then do not roll over to a 3-month period.

  • Pro-rations: No pro-rata refunds unless required by law.

  • Access Following Cancellation: Upon cancellation, you will continue to have access to your digital services and subscription features until the end of your notice period, provided all amounts due for that period have been paid. Access will end once your membership term expires.

  • Cancellation methods: You can cancel your subscription by precise cancellation methods (web form/email/telephone/post).
    - Cancellation confirmation will be provided via email within 72 hours.

  • Any outstanding balance remaining after cancellation will remain payable. We reserve the right to pursue recovery of unpaid amounts, and outstanding charges may be referred to debt management or collection processes.

Refunds

  • Eligibility: You may request a refund provided you have not attended your Annual Baseline Assessment, used your included GP consultation, or attended any included Healthspan Coaching session.

  • Exclusions: No refund will be available once you have attended your Annual Baseline Assessment, used your included GP consultation, or attended any included Healthspan Coaching session.

  • Timing: Approved refunds will be processed within 7 working days to the original payment method.

  • Partial service: Where you cancel before attending your Annual Baseline Assessment, GP consultation, or Healthspan Coaching sessions, any refund may be reduced to reflect the value of digital services, content, and other membership benefits already provided.

User conduct and service suspension

  • No user-generated content: There are no public posts or forums.

  • Prohibited behaviour: Offensive or aggressive behaviour is prohibited.

  • Suspension/termination: We may suspend or terminate your account for offensive/aggressive behaviour or for missed payments. See also our Zero Tolerance Policy **[here](____________________

Data protection and privacy

Services levels, maintenance and outages

  • Service levels: No service level commitments are offered.

  • Interruptions: Maintenance may cause temporary interruptions; we shall seek to minimise disruption.

Warranties and liability

  • Nothing in these Terms limits or excludes out liability for: a) death or personal injury caused by our negligence; b) fraud or fraudulent misrepresentation; or c) any other liability which cannot be limited or excluded under applicable law. Nothing in these Terms affects your statutory rights.

  • We will provide the services with reasonable care and skill. If we do not, you may be entitled to remedies under the Consumer Rights Act 2015 (included repeat performance or a price reduction). Where we supply digital content, it will be as described and of satisfactory quality, and we will comply with our obligations under the Consumer Rights Act 2015.

  • No medical advice / not emergency care: BHM provides screening, information and coaching support only. It is not a substitute for medical diagnosis or treatment. If you have symptoms, concerns, or need urgent care, you should contact your GP or NHS 111 (or 999 in an emergency). You must not rely on BHM results or content as the sole basis for medical decisions.

  • While we take reasonable care to provide accurate results and information, assessments have inherent limitations and results may be affected by factors outside our control (including sample quality, timing, and third-party laboratory processes). We do not guarantee that BHM will identify all conditions or risks.

  • BHM is for personal, domestic use. If you use it for any commercial or business purpose, we will not be liable for any loss of profit, loss of business, business interruption, or loss of business opportunity.

  • We are responsible for losses you suffer that are a foreseeable result of our breach of these Terms or our negligence. We are not responsible for losses that are not foreseeable.

  • We are not responsible for losses arising from: a) your failure to provide accurate information; b) your failure to follow reasonable instructions (including pre-appointment preparation); c) misuse of home test kits; d) sharing login details or allowing others to use your membership; or e) your failure to seek appropriate medical advice where recommended.

  • We are not responsible for delays or failures caused by events outside our reasonable control, including internet outages, app store/platform issues, or failures of third-party payment providers. This does not affect our responsibility to provide the services with reasonable care and skill.

  • Subject to clause 1 (non-excludable liability). where deemed fair and reasonable, our total liability to you for all losses arising under or in connection with your membership shall not exceed 12 months' fees paid.

  • We will take appropriate technical and organisational measures to protect your personal data. We are not responsible for losses caused by your failure to keep your account credentials secure or by unauthorised access where we have complied with our security obligations.

  • We may use third-party laboratories, clinicians and other suppliers to deliver parts of BHM (for example, sample analysis, clinical review, consultations and coaching). We remain responsible to you for providing BHM with reasonable care and skill, but we are not responsible for delays or failures caused by those third parties where we have taken reasonable care in selecting and managing them. Where you are asked to contract directly with a third-party provider, we will make that clear to you and their terms will apply.

  • No guarantee of outcomes or improvement. BHM is intended to help you understand and monitor aspects of your health. Because health outcomes depend on many factors outisde our control (including your medical history, lifestyle, adherence to recommendations and factors identified through screening), we do not guarantee any particular outcome, improvement, diagnosis, prevention of illness, or that you will achieve any specific results.

Complaints and dispute resolution

  • How to complain: You can complain online or by phone; please see our complaints procedure for escalation steps here.

  • Mediation/ADR: Mediation is not offered.

  • Governing law and courts: Laws of England and Wales; courts of England and Wales.

Changes to these Terms and to BHM

  • We may update these terms to reflect changes to BHM, legal or regulatory requirements, or safety/quality needs. Where changes are material, we shall provide at least 3 months’ notice, or less where required for urgent safety or quality reasons.

  • We may change the service, substitute products, remove content, or discontinue the service on at least 3 months’ notice, or less where urgent for safety/quality.

Automatic renewals - compliance and your rights

  • Ongoing agreement: Your membership has a minimum 12-month term; after the inital term, 3 months' notice is required to cancel.

Membership communications, renewals and your rights

Membership communications

  • As part of your BHM membership, we may send you service communications relating to your membership, including appointment confirmations and reminders, results notifications, account access, coaching and consultation reminders, payment notifications, renewal information and other important membership updates.

  • These communications form part of the services provided under your membership and are separate from marketing communications.

Renewal reminders

  • To help you manage your membership, we will send renewal communications before your membership anniversary date, including:
    - A reminder approximately 4 month before renewal.
    - A cancellation deadline reminder approximately 3 months before renewal.
    - A final reminder approximately 30 days before renewal.


  • These communications will include information about your renewal date, membership fees, any material changes to your membership and how to cancel.

Cancellation

  • You may cancel your membership using the cancellation methods made available by us, including online where available. Details of how to cancel will be provided in your renewal communications and through our Customer Services Team.

Changes to membership

  • Where we make a material change to your membership, pricing or these Terms, we will provide advance notice where required by law.

Cooling-off and statutory rights

  • Where applicable law provides additional cooling-off or cancellation rights in relation to subscription renewals, those rights will apply. Nothing in these Terms affects your statutory rights.

  • Opt-out: You can opt out by giving notice per section 10 (Cancellation).

  • DMCCA implementation: The Digital Markets, Competition and Consumers Act 2024 introduces enhanced obligations for subscription contracts. We shall implement all required notices, reminders, and easy?to?use cancellation mechanisms in line with commencement.

Important consumer information about cooling-off

  • Cooling-off right: You have 14 days from sign-up to cancel, provided you have not had your first appointment within that period. See terms provided above (see sections 6 and 10).

  • Digital content access: We do not seek a waiver for immediate access to digital content.

Sanctions and territories

  • We do not accept subscriptions from outside England, Wales, Scotland, or from sanctioned countries.

Launch timing and naming hygiene

  • Launch timing: BHM Membership is scheduled to launch in November 2026.

  • Naming: “Health Monitor” now refers only to the BHM membership. “Subscribe and Save” is separate and not BHM. For more information about the ‘Subscribe and Save’ product information here.

Definitions

  • Anniversary: The annual mark from your subscription start date.

  • Minimum term: The first 12 months of your subscription.

  • BHM: The membership described in sections 2–3 above.

  • "Annual Baseline Assessment" means the comprehensive health assessment included within your membership and undertaken at the start of each membership year, which establishes your baseline results for comparison with subsequent assessments and retests.
