Why cortisol testing matters – and how to do it properly

Here’s how cortisol tests can help you understand your stress levels.

Stress leaves a pattern in your body. Most of us know the feeling: the racing mind at 3am, the fatigue that sleep doesn't fix, and the sense that something's off even when your GP bloods look normal.

Cortisol, your body's main stress hormone, is often part of that story.

Here's how testing it properly can help, and why the way it's measured matters as much as the result itself.

Why one reading isn't enough

A single cortisol sample can vary a lot from day to day. That's why our cortisol profile doesn't rely on one. Instead, it takes multiple samples across the day – morning, midday, afternoon and evening – to map your diurnal cortisol slope: the natural arc of cortisol rising after waking and declining through the day.

It's not any single reading that matters. It's the shape of the curve.

That shape matters because it's a stable, repeatable pattern rather than a noisy one-off number.

A 2017 systematic review and meta-analysis found that flatter diurnal cortisol slopes were significantly, though modestly, associated with poorer health across a range of outcomes, including cardiovascular disease, immune and inflammatory dysregulation, and all-cause mortality.

Five things your cortisol rhythm can reveal

Here are just some of the things that proper cortisol tests can tell you about your body.

It shows your stress rhythm, not just a snapshot.

HbA1c and lipid profiles show the downstream effects of chronic stress – what it's already done to your blood sugar and cardiovascular system.

But cortisol mapping shows the upstream hormonal driver: whether your stress response system is working as it should, or running in a pattern linked to long-term health risk. Used together, they tell a fuller story.

It shows you a window into metabolic risk that blood tests alone miss.

Research has found that salivary cortisol was uniquely linked to increased waist circumference, as well as metabolic biomarkers including triglycerides, HDL cholesterol and HbA1c (a diabetes marker).

It provides a more complete cardiovascular risk picture.

Chronic disruption of the cortisol system is linked to cardiovascular disease through several pathways: increased visceral fat, insulin resistance, raised inflammation and abnormal lipid levels.

A lipid panel shows the result; cortisol mapping can show the hormonal process driving it.

It shows a direct link to type 2 diabetes risk.

Chronically elevated cortisol raises blood glucose by stimulating the liver to produce more sugar and reducing insulin sensitivity, a process that significantly raises the risk of type 2 diabetes over time.

HbA1c is an advanced diabetes marker used to track the damage already done; cortisol mapping can indicate whether the process causing it is still active.

It goes a layer beyond vitamin D for bone health.

Cortisol dysregulation affects bone mineral density directly, by suppressing the cells responsible for building bone.

Vitamin D status is an important marker, but it won't tell you whether elevated cortisol is actively working against your bones.

Why we look at cortisol alongside metabolic markers

Metabolic syndrome – a cluster of conditions including raised triglycerides, elevated blood glucose, high blood pressure and excess abdominal fat – significantly raises the risk of cardiovascular disease and type 2 diabetes.

Abnormal salivary cortisol patterns are associated with exactly these markers.

Reading your cortisol slope alongside your lipid and metabolic data builds a picture of whether your hormonal system may be contributing to metabolic risk, and what you can do about it.

Combining cortisol results with your mental health screening

We also include PHQ-8 and GAD-7 questionnaires – the gold-standard clinical screening tools used by GPs and mental health professionals worldwide to identify depression and anxiety.

These are validated instruments with robust sensitivity and specificity, used in the NHS and internationally to guide clinical decisions.

Cortisol dysregulation and mental health aren't separate stories. A disrupted cortisol slope is associated with both anxiety and depression – the same nervous pathway dysfunction between the brain and adrenal glands, where cortisol is made (HPA axis), is associated with mood disorders.

Looking at both together can surface patterns that neither the blood test nor the questionnaire would reveal alone.

Making sense of it with a health coach
